Assist individuals in transitioning from nursing facility care to the community as part of Money Follows the Person, Connecticut’s Medicaid rebalancing program. Entry level position for someone with broad range of social service skills along with organizational skills to manage complex housing, health and financial documentation. Firm commitment to the policies of self-determination and person-centered care. Ability to drive throughout Fairfield County and basic computer skills required. Required Associate Degree in Human Services or related field.
