Money Follows The Person

Money Follows the Person (MFP)

Money Follows the Person is a demonstration project funded by The Centers for Medicare and Medicaid Services and administered by The CT Department of Social Services. MFP assists nursing home residents who wish to transition back into the community to live independently. If you or a loved one are a resident in a nursing home and wish to participate in Money Follows the Person, contact Money Follows the Person at The Department of Social Services at 888- 992-8637.

Basic Eligibility Criteria:

  • must have been a resident of a nursing home, hospital for at least three
    months
  • must have been on Medicaid at least one day prior to date of transition
  • must have a desire to return to the community to live independently

What MFP provides:

  • access to one of seven Medicaid home and community based waivers
  • access to Rental Assistance program
  • Home modifications
  • Assistive Technology

What happens after a referral is made?

After a referral is made to the CT Dept. of Social Services, your case will be assigned to a transition coordinator located at either The Area Agency on Aging or a Center for independent Living. The Transition Coordinator will come out to meet with the individual interested in transitioning and develop a person centered transition plan with the consumer. A care planner from one of the Medicaid waivers may also be involved to help develop the care plan based on the services an individual might need. Assistance with locating housing is also available through a Housing Coordinator who is part of the transition team.
