Transition Coordinator – Assist individuals in transitioning from nursing facility care to the community as part of Money Follows the Person, Connecticut’s Medicaid rebalancing program. Must have a broad range of social service skills along with organizational skills to manage complex housing, health and financial documentation. Firm commitment to the policies of self determination and person-centered care. Bachelor’s Degree in Human Services or related field. Specialized Care Manager – Join the Money Follows the Person team by engaging with clients to help them transition from a nursing facility back to the community. Excellent interviewing and assessment skills a requirement. Ability to develop care plans while following complex Medicaid and State funding requirements. Able to work as part of a multi-disciplinary team to advocate and support client goals. Ability to drive throughout Fairfield County and basic computer skills required. Care Manager (Stamford region) – Social Service Professional to assist older adults in maintaining maximum independence and dignity by assessing needs and developing a safe, effective plan of care to enable the client to remain in the community and forego nursing home care. Able to work as part of a multi-disciplinary team to advocate and support client goals. Ability to drive throughout Fairfield County and basic computer skills required.
