Care Manager – Greater Stamford, CT area. Social Service Professional to assist older adults in maintaining maximum independence and dignity by assessing needs and developing a safe, effective plan of care to enable the client to remain in the community and forego nursing home care. Able to work as part of a multi-disciplinary team to advocate and support client goals. Ability to drive throughout Fairfield County and basic computer skills required.
