Current Positions:

 

Care Manager – Social Service Professional to assist older adults in maintaining maximum independence and dignity by assessing needs and developing a safe, effective plan of care. The program goal is to enable the client to remain in the community and forego nursing home placement. Candidates must be able to work as part of a multi-disciplinary team to advocate and support client goals. Basic computer skills and the ability to drive throughout Fairfield County required. Specialized Care Manager- Highly skilled LCSW, LMSW or nurse to provide assessment and care management services for older adults and persons with disabilities in the community. This Medicaid-funded program provides an alternative to nursing home placement and seeks to maximize the quality of life for individuals with complex medical, psychological and/or cognitive issues. The ideal candidate will have excellent interpersonal and problem-solving skills while utilizing a person-centered philosophy to support clients. Candidates must have basic computer skills and the ability to drive throughout Fairfield & New Haven County. Associate Care Manager- Social service professional to initiate services and monitor care plans for individuals with Alzheimer’s and other types of dementia. Position also supports a veteran’s self directed care program. Join an innovative CT Medicaid contractor responsible to help older adults and persons with disability maximize their quality of life and independence. The Southwestern CT Agency on Aging provides person centered care management to thousands of residents in the region. The Agency seeks a Master’s prepared Social Worker with experience in case management to support quality assurance and training efforts and to oversee the internship program.
