Current Positions:


Specialized Care Manager- Highly skilled LCSW, LMSW or nurse to provide assessment and care management services for older adults and persons with disabilities in the community. This Medicaid-funded program provides an alternative to nursing home placement and seeks to maximize the quality of life for individuals with complex medical, psychological and/or cognitive issues. The ideal candidate will have excellent interpersonal and problem-solving skills while utilizing a person-centered philosophy to support clients. Candidates must have basic computer skills and the ability to drive throughout the shoreline of Fairfield County and New Haven County area (East Lime to Greenwich).
