SWCAA performs a needs assessment and provides education, information and community referrals in support of older people and people with disabilities. The Southwestern CT Agency on Aging helps seniors and people with disabilities make informed choices about their health, insurance, housing and long-term care. The Agency supports natural caregivers by providing temporary care and assistance to eligible individuals to give them a well-deserved rest. SWCAA Care Managers are social service professionals who help assess and develop a person-centered care plan designed to help the elderly, people with disabilities, and people with acquired brain injuries.
