How does one apply?

To apply for the Connecticut Homecare Program for the Elderly (CHCPE), or to refer an elder to this program please call the Alternate care Unit at the CT Department of Social Services at (800) 445-5394 or (860) 424-4904.

When calling please have the following information available to you with regards to the person you are referring:

• their name, date of birth, address and phone number
• the person's monthly income
• the person's liquid assets
• Social Security number
• Medicaid number (if applicable)
• General idea of their health care status

Upon referral to the Alternate Care Unit, your case will be referred to a Care Manager in their region who will arrange to do a homevisit and comprehensive assessment of the individual needing services to determine functional and financial eligibility and develop an appropriate care plan with the individual and/or their family.
