Money Follows the Person (MFP)

Money Follows the Person (MFP) is a demonstration project funded by The Centers for Medicare and Medicaid Services and administered by The CT Deptartment of Social Services.  MFP assists nursing home residents who wish to transition back into the community to live independently.  If you or a loved one are a resident in a nursing home and wish to participate in Money Follows the Person, contact Money Follows the Person at The Department of Social Services; 1(888) 992-8637.
