Home Care Agencies
Skilled home care agencies provide a variety of services in the home to keep an individual safe in the community and prevent premature nursing home placement. Services include visiting nurses, social workers, physical or occupational therapists and home health aides. Home health aides can provide hands on personal care such as bathing, dressing, grooming, assistance with meals, ambulation, transferring and other hands-on care.
All of the following agencies are contracted with SWCAA to provide services through either the CT Homecare Program, the Statewide Respite Care Program and National Family Caregiver Program.
