MFP Team Assistant. Organized, personable, computer-oriented administrative staff needed for full time position in busy human services department. Compile and enter data from multiple users in multiple formats. Applicant must be able to review and analyze data for accuracy; draft reports in response to funder requirements; and identify trends to alert staff of potential issues. Care Associate – Bilingual (English – Spanish), Bachelor’s Human Services, or related field. Entry level position to support the care management of elder clients in a community-based program. Assist clients telephonically by reviewing service needs and assigning providers. Answer calls and assist care managers with thorough documentation of contacts. Experience with evaluation, care plan development and person-centered goal planning required. Highly organized, excellent verbal and written communication skills. ABI Specialized Care Manager-, Highly skilled LCSW, LMSW, Masters of Counseling or nurse to provide assessment and care coordination services for individuals with Acquired Brain Injury. Program provides an alternative to nursing home placement and seeks to maximize the quality of life for individuals with complex medical, psychological and/or cognitive issues. The ideal candidate will have excellent interpersonal and problem-solving skills while utilizing a person-centered philosophy to support clients. Territory spans from Bridgeport to Middletown. Community Health Worker Opportunity- The Southwestern CT Agency on Aging has a part time position for a Community Health Worker to provide nutrition education individuals in the Home Delivered Meal Program throughout the 14 southwestern CT cities and towns. The ideal candidate will intake potential clients for the meals, establish eligibility and educate caregivers and clients on nutrition and community resources. The Community Health Worker will also monitor community meal sites and home delivered meal routes in the region. Flexible, 20 hours/week. Combination of office and community-based work.
